In this course, attendees will learn how to identify root causes, contributing factors, and systemic issues behind workplace incidents, accidents and near misses.

Participants will learn:

Introduction to Incident Investigation Process (Understanding):

Participants will gain an understanding of the incident investigation process, starting from the initial stages of gathering evidence and witness statements to the advanced steps of analyzing data and determining underlying causes.
Focus on Objective Investigations (Application):

Emphasis will be placed on the application of conducting objective investigations without blame, fostering a focus on preventing future incidents rather than assigning fault.
Incident Classification (Understanding):
Participants will categorize incidents, demonstrating their understanding of different incident types.
Data Collection Methods (Application):
Application of various methods for collecting incident data will be explored, allowing participants to apply knowledge in practical scenarios.
Evidence Preservation (Application):
Participants will apply techniques for preserving evidence, ensuring its integrity for thorough investigations.

Application and analysis of various root cause analysis tools, including:
The 5 Whys
Fishbone (Ishikawa) Diagram
Fault Tree Analysis
Development of Comprehensive Reports (Application):

Participants will learn how to develop comprehensive incident investigation reports, applying their knowledge to communicate findings effectively.
Recommendation of Corrective and Preventive Actions (Application):

Application of skills to recommend effective corrective and preventive actions will be a focus, allowing participants to propose actionable solutions.
